Eligibility & Costs GOLD CARD DVA clients are eligible and require a doctor's referral and an accompanying D904 DVA form before treatment can commence. If you have had physiotherapy treatment recently then prior DVA approval may be required. This service is mostly available South of the River (please call 0408 908 056to confirm that your suburb is covered). The service is designed to treat DVA clients that are not physically able to leave the home or are unable to attend a regular physiotherapy practice due to transport difficulties.
